This workshop requires you to answer the following supplemental questions.

1. Part of our ITI experience will include online discussion prior to the actual Institute meeting. We will accomplish this discussion by inviting you to a Google Group. Which is your preferred email address during the summer months?

2. In what grade level(s) and subject area(s) do you primarily teach?

3. The prerequisite for attending the ITI-DMW2 is participation in the beginning four-day ITI-DMW in 2003-2006, the training for DV Voice, the one-day Screen Education Overview from Summer 2007, the CUE 2008 Conference, or a variety of other one-day Overview workshops offered by AFI. You may also attend the August 11, 2008 Screen Education Overview to fulfill this requirement. If seats are available, repeat attendees of the ITI-DMW2 will be considered. Describe your prerequisite experience and the date you attended or plan to attend.

4. The ITI-DMW2 is a leadership training, although it is realized that selected Fellows will come to the Institute with a variety of skill sets and levels of experience. You will leave the ITI having gone through a unique hands-on experience with resources to share. It is the hope of the Institute organizers that you share this information with students and/or teachers at your school site or within your district. Please briefly share your purpose and commitment for wishing to attend the ITI-Digital Multimedia Workshop 2. Your response need not be lengthy, but it should give the reviewers enough information to understand your background, qualifications, dedication, and specifically, how you will use the information and skills gained in the 2008-2009 school year with your colleagues and students. It is suggested that you compose your answer in a word-processing document, then copy and paste the information into the online field.
